600 Application for Beneficial Water Use Permit (New 7/1/09)
For appropriations of groundwater of more than 35 gallons per minute or 10 acre-feet per year and all surface water. Submission of this application must include applicable addendums below.

For appropriations of 5.5 or more cubic feet per second and 4,000 acre-feet.
602 Notice of Completion of Groundwater Development
For groundwater developments with a maximum use of 35 gallons per minute or less not to exceed 10 acre-feet per year.
603 Well Log Report Used only by Drillers to Record Wells Drilled
605 Application for Provisional Permit for Completed Stockwater Pit or Reservoir
May be used for a completed stockwater pit or reservoir when: (1) the only purpose is stockwater (2) the storage capacity is less than 15 acre-feet with an annual appropriation of less than 30 acre-feet per year, (3) the reservoir is located on a nonperennial flowing stream, and (4) and it is constructed on and accessible to a parcel of land 40 acres or larger and owned or under the control of the applicant.
606 Application for Change of Appropriation Water Right (New 7/1/09)
For any change in point of diversion, place of use, purpose of use, or place of storage of any water right. Submission of this application may require one of the following criteria addendums. See form Nos. 606-TEMP,
606-ST, 606B, or 606ASW.

606-TEMP Temporary Change Addendum
606B Supplement to Application for Change of Appropriation Water Right
For any change in purpose of use or place of use of 5.5 or more cubic feet per second and 4,000 or more acre-feet.
606ASW Supplement to Application for Change of Appropriation Water Right
For salvage water.
607 Application for Extension of Time
Request for additional time to complete a Provisional Permit or Authorization Change.
608 DNRC Water Right Ownership Update
Used for all changes in ownership of entire water right which took place prior to July 1, 2008 (including change of entire water right in connection with a real estate transfer). Do not use this form if you are a user of a municipal or irrigation district water system. For information regarding post July 1, 2008 transfers, please visit our website at www.dnrc./wrd/water_rts/HB39/default.asp.
Form No. 641, for a water right that will be divided.
Form No. 642, for a water right that will be exempted (reserved) from a sale of land.
Form No. 643, for a water right that will be severed from land. A severed water right does not involve a land sale.
